September 28, 2012 ATTENTION OWNERS
To serve you more efficiently, effective November 1, 2012, the Fort Worth Housing Authority is implementing a self- certification process for certain Housing Quality Standards (HQS) repairs. In lieu of a second inspection, eligible owners will now self-certify to the completion of HQS repairs for non-emergency deficiencies cited during Annual, Complaint, Special, and/or Quality Control inspections. The self-certification process is not an option for Initial inspections.
The self-certification is in place of a second physical inspection being conducted. No reinspection will be scheduled or conducted. You (the owner) will receive written notification and instructions if you are eligible. The owner and tenant will sign a certification form indicating that repairs have been completed. Owners may also be required to submit proof of repairs such as receipts, invoices, or photos. If the certification is not received (or is incomplete) by the date due, the HAP contract will be terminated/abated. Emergency fails will continue to be reinspected within 24 hours.
Quality control checks will be conducted on a certain number of self-certifications. If it is determined that an owner has not been truthful on the self-certification form, the owner will be prohibited from participating in the Housing Choice Voucher Program. The tenant’s assistance may also be jeopardized if the tenant was not truthful on the form.
Decent, safe and sanitary units are critical to the success of the Housing Choice Voucher Program. FWHA strongly encourages you to make any necessary repairs prior to the first scheduled inspection. Owners who have a high rate of failed first inspections may not be able to participate in the self-certification process.
If you have any questions, you may email me at smitchell@ftwha.org.
